The Puppet Master form can only be obtained from Puppeteers obtained from the Puppeteer Egg Box item. This form is not inheritable.
The Teatime form is a special form that can only be obtained from breeding two Puppet Master form Puppeteers together. The offspring will have a 1/6 (or 16.67%) chance of being the Teatime form when it grows to adult. This form is not inheritable, but is tradeable before it grows.
You won't know whether the Puppeteer will grow into the Teatime form until it reaches the adult stage.
The Default form is obtained from breeding.
